Southern Labware Products Decontamination Form

The Southern Labware Products Decontamination Form is used to protect the health and safety of Southern Labware Products Employees. We require that all products be fully decontaminated prior to return for warranty or repair. Please fill out this form and submit it to customerservice@southernlabware.com. The decontamination form must be submitted for review and authorization and reviewed by the Southern Labware Product Manager. Once approved, the Product Manager will provide a signed and authorized form to accompany the return. Return/warranties will not be accepted or fulfilled unless the decontamination form has been signed and authorized.
